Short Absence Form

If you need to take your child out of school during the school day for things like an appointment or exam, we ask that this online form is completed in advance of the day the time requested out of school. This form is then received by the office and your child's class teacher is made aware.

When possible, please complete this form for each appointment as far in advance as possible.

Medication

If you require medication to be administered to your child during the school day, please complete the relevant form and hand it into the office, along with the medication.  Please refer to our Administration of Medication policy for further details of the circumstances in which we will administer medication.
